Visit Reason
The survey was conducted as a renewal inspection of the assisted living facility.
Findings
Four deficiencies were identified related to oxygen storage security, food service meal provision, resident needs assessments, and resident service plan updates following hospital discharge.

37.106.2853-4 Oxygen use: the second floor oxygen storage room had ten unsecured oxygen cannisters.
37.106.2860-2 Food service: two of four resident files indicated service for two meals per day, which was by choice and additional meals could be provided at extra cost.
37.106.2821-3 Resident application and needs assessment: two of four residents did not have a needs assessment documented prior to admission.
37.106.2822-3 Resident service plan: category A: a resident returning from hospital with a toe amputation had no change in service plan to address the condition or services provided.
