The facility failed to ensure residents are free from all forms of abuse including physical abuse, resulting in a resident sustaining bruising on the right forearm. The facility suspended and terminated the employee involved and provided retraining and re-education on abuse prevention to all staff.

The facility committed to retrain staff on Abuse and Neglect and Resident Rights policies, conduct in-service training for all staff by July 31, 2025, and encourage open communication to prevent abuse or neglect.
