An on-site investigation of complaints 00162354 and 00162380 was conducted on April 3, 2026, and the following deficiency was cited:
Findings
The inspection found one deficiency related to failure to ensure personnel provided evidence of freedom from infectious tuberculosis as required. This posed a potential TB exposure risk to residents.

One deficiency was found related to incomplete documentation of services provided in a resident's medical record.

A manager failed to ensure a caregiver or assistant caregiver documented the services provided in the resident's medical record for one of two residents sampled. The service plan required catheter care twice a day, but documentation showed it was only completed once a day despite staff reporting the service was provided as planned.
