Findings
Two deficiencies were found related to the facility's failure to ensure proper means of egress alerting and incomplete disaster drill documentation. The facility provided plans of correction for both deficiencies.

The manager failed to ensure the means of exiting the facility for residents without keys or special knowledge included a method to alert employees of egress. The patio door to the backyard did not have a means of alerting staff when residents exited.
The manager failed to ensure disaster drills for employees were conducted on each shift at least once every three months and properly documented. Documentation showed drills last conducted in December 2022 with no recent records available.
