Findings
L & W Adult Foster Care Home is wheelchair accessible with two ramps at two approved means of egress. Resident bedrooms located upstairs are not wheelchair accessible and residents with impaired mobility shall not be assigned those bedrooms.

Findings
The home was found to be in non-compliance with 6 rules; a written corrective action plan was required before the license is renewed.

R 400.14301 Resident admission criteria; resident assessment plan; emergency admission; resident care agreement; physician's instructions; health care appraisal: the home is not wheelchair accessible as it does not have two approved means of egress from the first floor, but Resident R and Resident P now require the use of a wheelchair.
R 400.14310 Resident health care: there were no weights on file for Resident R because the home does not have a wheelchair accessible scale and Resident R is unable to stand on an average scale.
R 400.14312 Resident medications: Resident K was given 2 tablets of Ferrous Sulfate 325 mg daily from 05/01/23 to 05/09/23 instead of the prescribed 1 tablet daily. Repeat violation.
R 400.14312 Resident medications: the reason for each administration of Resident K's as-needed Albuterol inhaler was not recorded. Repeat violation.
R 400.14312 Resident medications: Resident P's Albuterol inhaler was stored in Resident K's medication box, failing to take reasonable precautions to ensure medication is not used by a resident for whom it was not prescribed.
R 400.14509 Means of egress; wheelchairs: the home is not equipped with ramps at two approved means of egress from the first floor; it has only one ramp off the front door, but Resident R and Resident P require wheelchairs.
