Findings
The inspection found seven deficiencies related to staff training, documentation, and food safety practices. The facility failed to provide required training, maintain proper documentation, and ensure food safety measures were followed.

36-420.01 — The manager failed to ensure the facility administered a fall prevention and fall recovery training program for all staff, as documentation for one personnel was missing. This posed a health and safety risk for residents.
Skills verification — The manager failed to ensure that a caregiver's skills and knowledge were verified and documented before providing health services for two personnel. This posed a risk if personnel were unable to meet resident needs.

The manager failed to ensure medication was stored in a separate locked room, closet, cabinet, or self-contained unit. Medications including Amlodipine 2.5mg, Senna 8.6mg, Hydralazine 50mg, and Diphenhydramine 25mg were unlocked and accessible to residents.
The manager failed to ensure poisonous or toxic materials were maintained in a locked area and inaccessible to residents. An unlocked kitchen cabinet contained substances such as Multi-Surface Cleaner With Bleach, Weiman Stove & Oven Cleaner, Comet Bleach, Clorox Fraganzia, and Weiman Glass Cook Top Cleaner & Polish.
