36-420.01 — The manager failed to ensure the facility administered a fall prevention and fall recovery training program for all staff, as documentation for one personnel was missing. This posed a health and safety risk for residents.

Skills verification — The manager failed to ensure that a caregiver's skills and knowledge were verified and documented before providing health services for two personnel. This posed a risk if personnel were unable to meet resident needs.

Skills documentation — The manager failed to maintain documentation of caregivers and assistant caregivers working each day, including hours worked. This posed a risk as there was no record of staff presence to ensure resident safety.

CPR and first aid training — The manager failed to ensure personnel records included documentation of required cardiopulmonary resuscitation and first aid training for one personnel. This posed a risk during emergencies.

F.2 — The manager failed to ensure a means of exiting the facility that controlled or alerted employees of resident egress was active. Alarms were present but not turned on, posing a risk of unmonitored resident egress.

Food temperature — The manager failed to ensure potentially hazardous foods requiring refrigeration were maintained at 41° F or below. Several sauces requiring refrigeration were stored at room temperature, risking foodborne illness.

Food storage thermometer — The manager failed to ensure the refrigerator used for food storage contained a thermometer accurate to plus or minus 3° F placed at the warmest part. The observed refrigerator temperature was 50° F.

The manager failed to ensure medication was stored in a separate locked room, closet, cabinet, or self-contained unit. Medications including Amlodipine 2.5mg, Senna 8.6mg, Hydralazine 50mg, and Diphenhydramine 25mg were unlocked and accessible to residents.

The manager failed to ensure poisonous or toxic materials were maintained in a locked area and inaccessible to residents. An unlocked kitchen cabinet contained substances such as Multi-Surface Cleaner With Bleach, Weiman Stove & Oven Cleaner, Comet Bleach, Clorox Fraganzia, and Weiman Glass Cook Top Cleaner & Polish.
