Visit Reason
The Department conducted a life safety survey at Maple Ridge Manor to assess compliance with fire safety regulations.
Findings
The facility failed to perform quarterly fire drills on all shifts for the fourth quarter of 2018 and the first quarter of 2019. The Administrator verified that fire drills had not been conducted on the specified shifts.

Rule 1200-08-25-.10(3)(a) requires quarterly fire drills for each ACLF work shift. The facility failed to conduct fire drills on the 1st and 2nd shifts for the fourth quarter of 2018 and the 1st shift for the first quarter of 2019.
