The inspection was conducted as a Renewal Licensing Study to evaluate compliance with licensing requirements and to determine if the facility license should be renewed.

Findings
The facility was found to be in non-compliance with food safety rules due to unlabeled and undated opened food items in the walk-in refrigerator and improper storage practices such as scoops left in food bins.

Food items in the walk-in refrigerator were opened and stored without labels or dates, making it unknown how long the food was stored. Scoops were left in a flour bin and a food storage bin.

The purpose of this addendum is to document an increase in licensed bed capacity from 54 to 83 by converting 30 single occupancy resident rooms to double occupancy rooms.

Findings
The facility was reviewed and found to have adequate room sizes, dining, activity, and window space to accommodate the increased licensed bed capacity. All resident rooms are ADA accessible and equipped with necessary safety features.
