Complaint Details
The following deficiencies were found during the on-site compliance inspection and investigation of complaint AZ00215152, AZ00220286, and AZ00219609 conducted on February 11, 2025.
Findings
The inspection found four deficiencies related to incomplete or inaccurate resident service plans, inadequate documentation of services provided, altered medical record entries, and medication administration without proper medical practitioner authorization.

R9-10-808 — The manager failed to ensure a resident's written service plan included an accurate description of the resident’s medical or health problems, including physical, behavioral, cognitive, or functional conditions.
R9-10-808 — The manager failed to ensure a caregiver documented the services provided in a resident's service plan, with missing documentation for mobility and dressing services.
R9-10-811 — The manager failed to ensure entries in a resident’s medical record were not altered to make the initial entries illegible, as evidenced by use of correction tape over dates and balances.
R9-10-816 — The manager failed to ensure medication was administered under the direction of a medical practitioner, as a required authorization document was signed by a registered nurse instead of the physician.
