Findings
Five deficiencies were found related to emergency responder patient information forms, designation of a qualified caregiver in the manager's absence, documentation of services provided, and medication administration records. The deficiencies posed risks including lack of emergency information, absence of designated management, incomplete service documentation, and inaccurate medication records.

36-420.04 — The manager failed to ensure standardized emergency responder patient information forms were completed and maintained for all five residents sampled, leaving required patient information unprepared for emergencies.
A manager designation failure — The manager did not ensure a qualified caregiver was designated in writing and present to be accountable when the manager was absent, leaving no individual responsible for onsite management.
A manager documentation failure — The manager failed to ensure caregivers documented services provided in residents' medical records for all five residents, preventing verification of services against service plans.
C.13.c — The manager failed to ensure residents' medical records contained the name and signature of the individual administering medication for four residents, resulting in unverifiable medication administration and misleading information.
B.3.c — The manager failed to ensure medication administration was accurately documented in residents' medical records, including instances of medications documented as administered before the actual administration date and invalid dates.
