Visit Reason
On-site compliance (annual) inspection of an Assisted Living Home conducted on May 21, 2025.
Findings
Six deficiencies were found related to quality management, medication storage, emergency drills, evacuation drills, resident orientation, and storage of poisonous materials. The facility failed to meet several regulatory requirements designed to ensure resident safety and proper management.
Deficiencies (6)
R9-10-804 — The manager failed to ensure a documented quality management report was submitted to the governing authority identifying concerns about resident care and actions taken. No documentation was provided for Compliance Officer review.
R9-10-816 — The manager failed to ensure medication was stored in a separate locked area; a medication cabinet was observed unlocked and accessible to residents. Caregivers acknowledged the medication was stored unlocked.
R9-10-818 — The manager failed to ensure disaster drills for employees were conducted on each shift at least once every three months and documented. Documentation was incomplete and acknowledged missing by staff.
R9-10-818 — The manager failed to ensure evacuation drills for employees and residents were conducted at least once every six months. No documentation of evacuation drills was available for the last 12 months.
R9-10-818 — The manager failed to ensure one resident received orientation to the facility exits and evacuation route within 24 hours of admission. The resident's record lacked documentation of this orientation.
R9-10-819 — The manager failed to ensure poisonous or toxic materials were stored in a locked area inaccessible to residents. Cleaning supplies were found in unlocked cabinets accessible to residents.
