Visit Reason
On-site compliance (annual) inspection of an Assisted Living Home conducted on June 18, 2025.
Findings
The inspection found eight deficiencies related to tuberculosis training, quality management, service plans, medication storage, and emergency and safety standards. Plans of correction were provided for all deficiencies.

R9-10-113 — The facility failed to provide annual training and education related to recognizing the signs and symptoms of tuberculosis for three caregivers, posing a potential illness risk to residents.
R9-10-804 — The manager failed to implement the facility's quality management program, as no documented quality management report was available, posing a risk to effective management of resident services.
R9-10-808 — The manager failed to ensure a written service plan was available for one resident, posing a health and safety risk if caregivers did not know the services needed.
R9-10-808 — The manager failed to ensure a resident's written service plan was reviewed and updated at least once every six months, posing a risk if caregivers were unaware of updated services.
R9-10-816 — The manager failed to ensure medication was stored in a separate locked area; a medication cabinet was observed unlocked and accessible to residents, posing a safety risk.
