Visit Reason
The Office of the State Fire Marshal conducted a scheduled fire safety inspection at the Monroe Care residential care facility.
Findings
The inspection identified multiple fire safety violations related to equipment, power taps, extension cords, cleaning, and maintenance. All violations were noted as corrected except for one hold-open device violation where a door was held open with a door stop.
Deficiencies (11)
IFC 603.1.1 (2021) - All electrical equipment, wiring, devices, and appliances must be tested, listed, labeled, and maintained per NFPA 70 and instructions. This requirement was found to be corrected.
IFC 603.5 (2021) - Relocatable power taps and current taps must comply with NFPA 70 and the code. This violation was corrected.
IFC 603.5.2 (2021) - Relocatable power taps and current taps must be directly connected to a permanently installed receptacle with exceptions. This violation was corrected.
IFC 603.6 (2021) - Extension cords shall not substitute permanent wiring and must be listed and labeled per UL 817. Extension cords must not be affixed or exposed to damage and are for portable appliances only. This violation was corrected.
IFC 606.3.3 (2021) - Hoods, grease-removal devices, fans, ducts, and appurtenances must be cleaned at required intervals. This violation was corrected.
IFC 701.6 (2021) - The owner must maintain an inventory of required fire-resistance-rated construction and inspect and repair annually. This violation was corrected.
IFC 705.2.2 (2021) - Opening protectives in fire-resistance-rated assemblies must be inspected and maintained per NFPA 80 and 105. This violation was corrected.
IFC 705.2.3 (2021) - Hold-open devices and automatic door closers must be maintained and doors remain closed when device is out of service. The dietary office door was held open with a door stop, which is a violation.
IFC 903.5 (2021) - Sprinkler systems must be tested and maintained per Section 901. This violation was corrected.
IFC 906.6 (2021) - Portable fire extinguishers must not be obstructed or obscured and locations must be indicated where visual obstruction cannot be avoided. This violation was corrected.
IFC 907.4.2.6 (2021) - Manual fire alarm boxes must have ready access, be unobstructed, unobscured, and visible at all times. This violation was corrected.
