Findings
One deficiency was found related to the lack of a secure means of exiting the facility for a resident that allows exit to a location at least 30 feet away with monitoring or alerting.

R9-10-815 — The manager failed to ensure there was a means of exiting the facility for a resident that allowed the resident to exit to a location at least 30 feet away from the facility that is secure. A door from a resident room leading to the backyard had no alert, was not locked, and had no monitoring system in place.

Findings
Two deficiencies were found related to personnel tuberculosis testing and improper storage of toxic materials. The facility provided plans of correction for both deficiencies.

R9-10-806 — The manager failed to ensure that one of three personnel sampled provided evidence of freedom from infectious tuberculosis on or before the date the individual began providing services, lacking documentation of a second TB skin test.
R9-10-819 — The manager failed to ensure poisonous or toxic materials were maintained in labeled containers in a locked area separate from food preparation, dining areas, and medications, as an unlocked bathroom cabinet contained unlabeled and accessible toxic materials.
