Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on July 31, 2025.

Incident reporting · Emergency drills: Documentation of an elopement drill was missing for all shifts for the calendar years 2023, 2024 and 2025 reviewed, and documentation of a Fire drill was missing for the second and third shifts for calendar years 2023, 2024 and 2025 reviewed.
Quality assurance program: Documentation of an Evidenced Informed Falls Prevention Program review was missing for 2023 and 2024, documentation of audit details was missing for 2024 and 2025, medication was stored unsecured in a resident’s bathroom, family filled medication planners lacked required labels, and assessments by a physical/occupational therapist every six months were missing for four residents (repeat finding).
