Visit Reason
The Minnesota Department of Health completed a survey to evaluate and assess compliance with state licensing statutes for New Perspective Prior Lake.

Findings
The facility was found to be in substantial compliance but had multiple deficiencies including food code violations, infection control issues, missing emergency postings, privacy concerns during medication administration, fire safety code violations, incomplete staff training, medication storage and administration errors, and background study non-compliance.

Deficiencies (12)
Food was not prepared and served according to the Minnesota Food Code, including a Priority 1 violation for sanitizing equipment and utensils.
Failed to establish and maintain an infection control program consistent with accepted standards during medication administration.
Failed to post the 911 emergency number in common areas and near facility telephones.
Failed to protect resident records against unauthorized disclosure; computer screens with resident information left open and visible.
Failed to comply with State Fire Code including fire doors not closing and latching automatically, smoke alarms over 10 years old, liquid oxygen storage issues, and non-functioning emergency exit lights.
Failed to develop and maintain a complete fire safety and evacuation plan and provide required staff training.
Failed to ensure background study was submitted and received for one employee affiliated with this license.
Failed to ensure annual training included all required topics for one unlicensed personnel.
Medication cart was left unlocked and unattended, allowing unauthorized access.
