Findings
The facility was cited for failure to comply with the Health Care Worker Background Check Act by not providing annual employment verification for one employee. The facility also failed to ensure resident service plans were reviewed annually and did not address the level of service for eating for two residents. Additionally, the establishment did not employ or contract with a registered dietician despite providing therapeutic diets.
