Visit Reason
The inspection was a biennial survey to assess compliance with the Regulations Governing the Licensing and Functioning of Assisted Housing Programs for a Level III Residential Care Facility.
Findings
The facility was found not in substantial compliance due to multiple repeat deficiencies including unsafe medication storage, lack of routine maintenance and housekeeping, and failure to store food in a safe and sanitary manner.

Medications and treatments were not stored safely in a locked cabinet; medication cart was unlocked.
Facility failed to maintain cleanliness and timely repair of wear and tear; multiple ceiling tiles stained or missing, presence of spiders and bugs, brown substance on baseboards, and walls with marks and holes.
Food was stored in an unsafe and unsanitary manner; potatoes and juice stored on floor next to garbage and animal food, and refrigerator was dirty with smudges and brown spots.
