An on-site investigation of complaint AZ00200777 was conducted on September 19, 2023 and the following deficiencies were cited:
Findings
Two deficiencies were cited related to policies and procedures for resident safety and the functionality of the door alarm system. The inspection found risks to resident safety due to inadequate supervision and a disabled door alarm.
Deficiencies (2)
The manager failed to ensure policies and procedures were established and implemented to ensure the safety of a resident at high risk for wandering. Staff were unaware when the resident left the home unnoticed, posing a risk to resident health and safety.
The manager failed to ensure the door alarm system was active to alert employees of a resident's egress from the facility. The door alarm was found turned off, increasing risk to resident safety.

On-site compliance (annual) inspection of PENZI CARE HOME LLC conducted on August 9, 2023.
Findings
The inspection found four deficiencies related to resident restraints, cleanliness, hazardous materials storage, and use of resident sleeping areas as passageways. Plans of correction were provided for all deficiencies.

R9-10-101.199 — The manager failed to ensure residents were not subjected to restraints, as bed rails were used in the up position on two residents' beds restricting their movement and posing a safety risk.
The manager failed to ensure the premises were cleaned and disinfected according to policies, as soiled disposable incontinence pads were left on residents' beds producing a strong urine smell throughout the facility.
The manager failed to ensure hazardous materials were stored in a locked area inaccessible to residents, as an open and unlocked shed contained spray paint, brake fluid, paint, engine oil, and hydraulic fluid accessible to residents.
The manager failed to ensure a resident's sleeping area was not used as a passageway to another sleeping area, as a bedroom with two residents included a closet used as a caregiver's resting area and served as a passageway.
