The Department of Social and Health Services conducted a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies.

Findings
The follow-up inspection found no deficiencies and confirmed that the facility meets the Assisted Living Facility licensing requirements. Previously cited deficiencies were corrected as documented.

Deficiencies (6)
WAC 388-110-220 Enhanced adult residential care service standards. The facility failed to ensure staff had at least six hours of continuing education related to dementia and Alzheimer's disease and failed to ensure updated assessments for residents as required.
WAC 388-112A-0080 Completion of seventy-hour long-term care worker basic training was not verified for one of five sampled staff, placing residents at risk.
WAC 388-78A-2150 Signing negotiated service agreement. The facility failed to ensure the negotiated service agreement was signed annually by responsible parties for sampled residents.
WAC 388-78A-2100 On-going assessments. The facility failed to ensure qualified assessors completed ongoing assessments focused on residents' identified problems for sampled residents at risk for unmet care needs.
WAC 388-78A-2040 Other requirements. The facility failed to ensure fit testing for respirators was completed for five of five sampled staff, placing residents and staff at risk in the event of infectious disease spread.
