Findings
The establishment failed to orient new residents to emergency and evacuation plans within 10 days of arrival, ensure CPR-certified direct care staff were on duty at all times, complete initial health evaluations timely for new employees, provide entry TB screening for residents, and maintain proper food service sanitation including pest control and garbage storage.
