Direct care staff not trained or CPR/First Aid certified: not established. Staffing inadequate during third shift and weekends: established. Medications not disposed properly: not established. Resident A’s room unclean and smelly: not established.
Findings
One rule violation was established regarding insufficient direct care staff during third shift to meet resident needs. Three other allegations were not substantiated.
