Visit Reason
Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on August 2, 2024.

Findings
The state cited 9 findings, 8 of which were repeats from the previous review, exceeding the state median of 6. The action taken was a plan of correction required.

Emergency call response: In July 2024 there were 74 emergency call response times over the eight minutes required by the Residence policy, and documentation of hourly safety checks was incomplete (repeat finding).
Memory care unit: The laundry room door in the Special Care Residence unit was not secured according to the Residence safety procedures.
Resident care plans: Two Resident Records were missing documentation of a Service Plan reassessment within 30 days after residency began, and two records lacked documentation of six-month reassessments (repeat finding).
Quality assurance program: Staff were unfamiliar with SAMM policies, one medication marked for disposal was not removed, and one medication was not administered without explanation (repeat finding).
Incident reporting: The Residence filed 47 incident reports more than 24 hours late and failed to report some incidents to EOEA, with incomplete internal investigations (repeat finding).
Resident progress notes: Documentation of significant occurrences in Progress Notes was inconsistent for two Residents (repeat finding).
Resident records: The Residence did not consistently document all significant information in Correspondence Logs and missed documenting some 24-hour periods (repeat finding).
Staff training: Two records lacked documentation of Introductory Visits, and three PCA records were missing semiannual skills evaluations for 2024 (repeat finding).
Records given to the state: Monthly SAMM observation MARs were missing for six Residents (repeat finding).
