The Care Facility Carbon Monoxide Standards Act of 2016 requires carbon monoxide alarms within 15 feet of fossil-fuel burning devices. The home had no carbon monoxide alarms near two gas dryers and a gas stove.
The home held more than $200 for Resident 1 from December 2017 through December 2018 without documentation of assistance in establishing an interest-bearing account.
Emergency light boxes in the first and second floor lounges did not illuminate when test buttons were pressed.
On April 9, 2019, frozen ground meat and chicken patties were thawing on the kitchen countertop at 9:30am.
Lint accumulated in the lint traps of two dryers that were not in use and had no laundry inside at approximately 10:30am on April 9, 2019.
The home had not implemented safe use procedures for glucometers, including incorrect documentation and programming errors on multiple residents' glucometers.
