Life Safety

Visit Reason
The Office of the State Fire Marshal conducted a scheduled fire safety inspection at the facility on May 15, 2025.

Findings
The inspection found that sprinkler system testing is scheduled for June 5, 2025, and smoke detector sensitivity testing revealed 9 devices failed but were replaced, correcting 6 of them. The facility was approved with no open violations noted in this report.

IFC 903.5 (2021) - Sprinkler systems shall be tested and maintained in accordance with Section 901. Forward flow testing of the backflow preventers shall be required and is scheduled for June 5, 2025.
IFC 907.8.3 (2021) - Smoke detector sensitivity shall be checked within one year after installation and every alternate year thereafter. On the last fire alarm sensitivity test, 9 devices failed but were replaced, correcting 6 of them.

Visit Reason
The Department of Social and Health Services conducted a complaint investigation at Colonial Court Assisted Living and Memory Care based on complaint number 115225 regarding concerns about hygiene assistance, laundry services, bladder/urinary care, and staff responsiveness.

Complaint Details
The complaint investigation addressed four allegations: lack of hygiene assistance, lack of laundry services, lack of bladder/urinary care, and difficulty reaching staff to address concerns. Some allegations were substantiated with citations written, while others were not supported by findings. Technical assistance was provided regarding resident rights and discharge requirements.
Findings
The investigation found that residents were generally clean and groomed but the facility failed to monitor and address residents' resistive behaviors and update care plans accordingly. WAC 388-78A-2560 Administrator responsibilities. The facility failed to ensure the administrator or designee was available to address concerns related to care of sampled residents.
