Visit Reason

R 325.1913 Licenses and permits; general provisions: The home’s license was not posted in a conspicuous public area of the home.
R 325.1923 Employee's health: Employees #3, #4, and #5 did not have tuberculosis screening completed within 10 days of hire and prior to occupational exposure, and the home lacked a current annual TB risk assessment.
R 325.1953 Menus: The weekly menu was not posted.
R 325.1954 Meal and food records: The home lacked a meal census consistent with this rule for the preceding three-month period.
