State-compiled facility profile showing multiple inspections from 2021 to 2022 with deficiency history for Silvermark Assisted Living Plano.

Findings
The inspections revealed multiple violations including failure to maintain infection control policies and noncompliance with life safety standards such as hazardous area construction, emergency generator compliance, building repair, and gas pressure testing. Some violations were corrected while others remain pending.

The facility failed to establish and maintain an infection control policy and procedure designated to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of disease and infection.
The facility failed to ensure hazardous areas were constructed in compliance with licensing standards for assisted living facilities, and/or that volatile materials were not stored in the facility.
The facility failed to ensure an emergency generator was in compliance with licensing standards for assisted living facilities.
The facility failed to ensure the building was kept in good repair.
