Visit Reason
The inspection was conducted due to violations found related to fire drill evacuation times, improper medication disposal, and failure to develop and maintain a plan of care for residents.
Findings
The facility failed to ensure that three of fifty-one residents evacuated within the required thirteen minutes during a fire drill. Staff improperly disposed of medications in violation of facility policy. Additionally, the facility failed to develop and maintain a plan of care for each resident.

Visit Reason
