Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on October 16, 2024.

Resident care plans: Documentation of bed rail assessments confirming residents can independently navigate around bed rails was missing for three Residents for the calendar years 2023 and 2024.
Memory care unit: Chemicals were not secured in the common kitchen area and windows were easily manipulated to open fully in the Special Care Unit.
