The home was found to be in non-compliance with multiple rules; a written corrective action plan was required before the license is renewed.

R 325.1922 Admission and retention of residents: Residents A, B, and C had tuberculosis screening completed after their admission dates, not before.
R 325.1923 Employee's health: Employee 2 was hired on 11/19/24 but her tuberculosis screening was from a previous employer and completed on 2/26/24, not within 10 days of hire.
R 325.1932 Resident medications: Multiple residents missed scheduled medication doses without documented reasons on the medication administration records.
R 325.1979 General maintenance and storage: An exhaust fan in the visitor’s restroom was missing its cover, exposing fan blades; detergents and cleaning agents were unsecured on shelves in launderette rooms with doors propped open, posing ingestion and poisoning risks.
R 325.1981 Disaster plans: The facility's disaster plan lacked a general plan for an explosion, having only a plan for a bomb threat.

Findings
An administrative review revealed substantial compliance with the public health code and administrative rules regulating home for the aged facilities. The license has been renewed.

Findings
The bathing room is handicap accessible with emergency pull cords and supports the increased licensed bed capacity. It is recommended that the licensed bed capacity increase to 83.
