R 325.1932 Resident medications: Medication administration records were incomplete with blank entries for missed doses without reasons, despite follow-up documentation; staff failed to properly document medication passes.
R 325.1972 Solid wastes: Garbage containers throughout the facility, notably in the memory care kitchenette, were observed without lids.
R 325.1976 Kitchen and dietary: Perishable food items in the dry storage area were left uncovered in their original packaging after being opened.
