Visit Reason
Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on October 29, 2025.
Findings

Emergency call response: In the months of April and September 2025 there were 522 e-call response times over the 12 minutes required by the Residence policy (repeat finding).
Resident care plans: Documentation was missing that Residents have supervised access to a heating element, some service plan reviews within 30 days of residency commencement were missing, and assessments ensuring residents can independently navigate around bed rails were missing for three residents.
Incident reporting: The Residence filed 12 incident reports greater than 24 hours after the occurrence of the incident or accident (repeat finding).
Controlled medication handling: Age observed controlled substance medication storage procedures were inconsistent with Residence policy (repeat finding).
Resident records: The Residence did not use the Correspondence Logs to communicate all significant or pertinent information necessary (repeat finding).
Staff training: Documentation of General Orientation Requirements was missing for six personnel records.
