Complaint Details
The following deficiencies were found during the on-site compliance inspection and investigation of complaints 00136978, 00142339, 00145237, 00146553, 00146559, and 00146919 conducted on October 7, 2025.
Findings
The inspection found three deficiencies related to fall prevention training and emergency responder documentation. The facility failed to develop a comprehensive fall prevention training program, did not provide required written documentation to emergency responders for multiple residents, and lacked a standardized EMS form for residents.
Deficiencies (3)
A.R.S. § 36-420.01.A — The facility failed to develop a training program for all staff including initial and continued competency training in fall prevention and fall recovery. Documentation of such a program was not available at the time of inspection.
A.R.S. § 36-420.04.A.1-9 — The assisted living center failed to provide a written document containing all required information to emergency responders when contacting them on behalf of residents in four of five sampled cases. Documentation was not available for review for incidents involving residents R1, R2, R3, and R4.
A.R.S. § 36-420.04.C — The facility failed to maintain a standardized form for each resident including required information for emergency medical services. No such documentation was available for any of the five residents sampled.

The manager failed to ensure an evacuation drill for employees and residents was conducted at least once every six months, with no documentation of such a drill during the past 12 months.
The manager failed to ensure soiled linens were stored in a closed container away from food storage, kitchen, and dining areas, as uncovered soiled linen was observed in the kitchen.
The manager failed to ensure two dogs residing at the facility were licensed consistent with local ordinances, as no current licenses were provided.
