Complaint Details
The inspection was conducted in response to complaint intakes #GA00219014 and #GA00219015. The investigation started on 11/19/2021 and was completed on 12/27/2021.
Findings
The facility was found deficient in multiple areas including failure to ensure staff CPR certification within 60 days of employment, expired sprinkler system inspection tag, hot water temperature exceeding 120 degrees Fahrenheit, ineffective safety devices to prevent resident elopement, failure to complete annual competency reviews for certified medication aides, improper medication labeling and packaging, and failure to properly dispose of unused medications.

Staff hired to provide hands-on personal services did not have current CPR certification with return demonstration of competency within 60 days of employment for 1 of 5 sampled staff (Staff C).
Facility failed to comply with local fire code regarding sprinkler systems; expired inspection tag dated 9/2020 observed.
Hot water temperature exceeded 120 degrees Fahrenheit, measuring 124.6 degrees F in resident bathroom sink.
Facility failed to utilize effective safety devices to protect residents at risk of eloping; exit door keypad not working for weeks and temporary lock in place.
Certified medication aides did not complete annual competency reviews as required for 3 of 5 sampled staff (Staff B, Staff D, Staff E).
Medications for 2 of 5 sampled residents (Resident #1 and Resident #4) were improperly labeled and stored in bottles instead of unit or multi-unit dose packaging.
Facility failed to properly dispose of unused medications for 1 of 5 sampled residents (Resident #2); lorazepam 0.5 mg discontinued but still present with no documentation of disposal.
