Visit Reason
Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on April 5, 2024.
Findings
The state cited 7 findings, including 4 repeat findings, which is above the state median of 6. The action taken was a plan of correction required.

Food and dietary services: Current Resident menus were missing designations of which selections meet the daily recommend allowances of sodium, sugar and fat content as established by the Food and Nutrition Board of the National Research Council of the National Academy of Sciences.
Resident screening and assessment · Resident care plans: Four records lacked documentation confirming the initial assessment was conducted by a nurse, three records were missing assessment results for allergies and diagnosis, two records were missing dietary needs assessments, individualized enrichment activities documentation was missing in three special care residence records, and two residents using bed rails lacked required bi-annual physical/occupational therapist assessments (repeat finding).
Quality assurance program: Documentation of a bi-annual review of the Evidenced Informed Falls Prevention Program was missing for the 2022 calendar year (repeat finding).
Incident reporting: The Residence filed 49 incident reports greater than 24 hours after the incident or accident (repeat finding).
Resident progress notes: Inconsistent documentation of significant occurrences in the Progress Notes for two residents (repeat finding).
Resident records: The 90-day correspondence logs did not consistently document all significant or pertinent information necessary to maintain the continuity of care for all residents.
Staff training: Five records lacked documentation supporting that Introductory Visits were consistently conducted with all applicable staff prior to or within 48 hours after the provision of Personal Care services, at move-in or change in condition.
