Emergency call response: In September and October 2025 there were 45 emergency call response times over the 15 minutes required by the Residence policy, and documentation of hourly safety checks for October and November 2025 was missing during overnight hours (repeat finding).

Memory care unit: Staff training deficiencies included no response to triggered door alarms and operational hazards such as unsecured chemicals and access to heating elements in the Special Care Residence (repeat finding).

Resident screening and assessment · Resident care plans: Six resident records were missing documentation of initial nurse assessments and signatures of residents or legal representatives, and two records lacked timely service plan reviews.

Quality assurance program: Documentation of quarterly medication audits was missing required dates, targets, and responsible persons for 2025, and medication storage issues included loose pills, compromised blister packages, and unlabeled over-the-counter medications (repeat finding).

Emergency drills · Incident reporting: Documentation of missing person drills for the first and second shifts in 2024 was missing, and six incident reports were filed late (repeat finding).

Resident records: The Residence did not maintain Correspondence Logs for at least 90 days as required.

Staff training: Documentation was inconsistent for Introductory Visits and biannual skills evaluations for personal care staff, with missing records for some staff in 2024 and 2025 (repeat finding).
