Visit Reason
Biennial state compliance review of the assisted living residence by the Massachusetts Executive Office of Aging & Independence (AGE) on March 27, 2025.

Findings
The state cited 7 findings, which is above the state median of 6. The action taken was a plan of correction required.

Emergency call response: Documentation of the Residence’s management testing the emergency response system functionality as required by policy was missing for the 2024 calendar year.
Memory care unit: Documentation to support all required components of the Special Care Residence operations review for the first half of 2024 was missing, and heating elements in the common area were operational without required supervised access safeguards.
Resident care plans: Three records lacked documentation that Residents have supervised access to a heating element; two records lacked documentation of a Service Plan reassessment within 30 days of residency commencement; three records lacked documentation of personal emergency response devices or procedures; two records lacked documentation confirming description of services not affiliated with the Assisted Living Residence.
Quality assurance program: Documentation of audits for Service Planning, Safety Assurances, Medication Quality, quarterly medication documentation audits, and an Evidenced Informed Falls Prevention Program review were missing for the 2024 calendar year. Observations included unsecured over the counter medications, LMA medications not stored separately from SAMM, one over the counter medication not on the prescribed list, one expired medication, one medication without pharmacy label and open date, and one Medication Administration Record missing placement recording.
Staff training: Documentation supporting a training needs assessment in 2024 was missing. Disclosure forms and notification of the right to bring a Representative prior to the first meeting were not signed by the Resident, Resident Legal Representative, and/or Residence prior to move-in in six records.
Information given to residents: Missing Disclosure of Rights.
Records given to the state: Three Resident monthly safety check records and two Resident monthly LMA administration records were intermittently missing from move-in to the date of compliance review.
