Findings
The establishment failed to ensure two residents had appropriate fall interventions in their Service Plans and failed to document signatures on the plans. These failures contributed to fall incidents causing injuries, hospitalizations, and a substantial probability of severe harm.

A4010 (295.4010 Service Plan) - The establishment failed to ensure a resident with cognitive decline had fall interventions in her Service Plan and failed to update fall interventions after each fall incident for another resident; the Service Plans were not signed and dated by involved parties during care conferences.
