The assisted living facility must secure potentially hazardous supplies and equipment commensurate with the assessed needs of residents and their functional and cognitive abilities. The facility failed to secure manicure sets and housekeeping carts, placing all residents at risk.
WAC 388-78A-2462 Background checks Who is required to have. The assisted living facility must ensure that contractors who may have unsupervised access to residents have a Washington state background check. The facility failed to ensure a contracted employee had a completed background check, placing all residents at risk.
WAC 388-78A-2730 Licensee's responsibilities. The assisted living facility licensee is responsible for complying with applicable laws and rules. The facility failed to obtain a Medical Testing Site Waiver/CLIA certificate by the deadline, placing all residents at risk for receiving waived tests without proper certification.

Visit Reason
The Office of the State Fire Marshal conducted a fire safety inspection at the new 15-bed memory care facility, Legacy of Bothell, Phase II, to assess compliance with fire protection and life safety codes.
Findings
The inspection found multiple fire safety violations including missing sprinkler system documentation, missing escutcheon ring on a sprinkler head, improperly mounted fire extinguishers, absence of carbon monoxide alarms in several areas, and emergency exit doors equipped with prohibited key-operated dead bolts. The facility was disapproved and required to correct all violations upon re-inspection.

2016 NFPA 13 6.2.9.7 - The facility failed to provide a list of the sprinklers installed within the facility to be located within the sprinkler cabinet.
