Visit Reason
The visit was a biennial survey to assess compliance with regulations governing the licensing and functioning of a Level IV Residential Care Facility.
Findings
The facility was found not in substantial compliance due to deficiencies in sanitation and physical plant requirements, including failure to have the fuel heating system annually inspected and tagged, unsecured outdoor meshed fencing, and hot water temperatures exceeding the allowed maximum of 120°F.
