The complaint investigation (Compliance Determination #72700) was conducted from 02/10/2026 through 03/02/2026 regarding a report of a resident sustaining injury from care staff. The investigation found the facility failed to notify the department of the injury allegation, substantiating the complaint and resulting in citations.
Findings
The follow-up inspection on 04/23/2026 found no deficiencies and confirmed that the facility meets the Assisted Living Facility licensing requirements. Previously cited deficiencies were corrected.
