Findings
The facility had multiple violations in the most recent 2022 inspection related to staffing, training, resident assessments, medication counseling, food safety, and employee health screening. Earlier 2018 inspections revealed numerous life safety code violations including fire safety, sprinkler system maintenance, and building repair issues.

The facility failed to have sufficient staff to ensure safe evacuation of the facility in the event of emergency.
