Annual Licensure Survey. Violations cited in Sections 295.3000 Personnel Requirements, Qualifications and Training; 295.4000 Physician's Assessment; and 295.4050 Tuberculin Skin Test Procedures.
Findings
The establishment failed to ensure direct care staff had CPR training with return demonstration and adequate awake staffing. Annual physician assessments were not completed for three residents. The tuberculosis policy was outdated and incomplete, lacking required components.
