Findings
The establishment failed to verify an employee's nursing license with the Illinois Department of Professional Regulation and lacked a medication policy addressing assistance in self-administration of medication while providing supervision. These failures create a substantial probability of severe harm to all residents.

The establishment was found to have multiple deficiencies including failure to conduct required disaster preparedness drills, incomplete employee orientation and ongoing training, missing or unsigned physician assessments, incomplete resident service plans, and lack of a current tuberculosis infection control plan with required annual risk assessments.
