The surveyor conducted two complaint investigations at Touchmark on West Prospect related to alleged medication theft and unauthorized entry by unknown persons.

Complaint Details
Both complaints were substantiated. The medication theft complaint involved hydrocodone pills stolen on 12/19/2023, with a former caregiver suspected and terminated. The safe environment complaint involved unauthorized entry by unknown persons on 01/22/2024, facilitated by a former caregiver who gave entry codes.
Findings
Two violations of Chapter DHS 83 were substantiated: misappropriation of 60 hydrocodone pills by a former caregiver and failure to maintain a safe environment when a former caregiver allowed two unknown persons into the facility, resulting in theft of furniture.
