Emergency call response: In June 2023 there were 55 emergency call response times over the ten minutes required by the Residence policy, and in July 2024 there were 20 such delays.

Bed rail safety · Resident care plans: Documentation of a physical/occupational therapist assessment every six months was missing for three residents using bed rails, and two records lacked documentation of reassessment and service plan review every six months (repeat finding).

Quality assurance program: Documentation of a specific target date for follow-up and staff responsible for follow-up actions were missing for the first and second quarters of 2023. Additionally, one resident had a compromised blister pack and one had an undated open bottle of medication (repeat finding).

Incident reporting: The residence filed 27 incident reports more than 24 hours late during the period reviewed (repeat finding).

Controlled medication handling: The residence inconsistently documented controlled substance counts for one resident, had missing documentation of significant occurrences in progress notes for three residents, failed to use correspondence logs to communicate all necessary information, had seven personnel records missing influenza vaccine documentation, and two records lacked signatures confirming introductory visits within 48 hours of service provision (repeat finding).
