Visit Reason
The inspection was conducted as a renewal inspection to determine compliance with applicable licensing statutes and rules for Traditions of Saginaw - West.

Findings
The facility was found to be in substantial compliance with the public health code and administrative rules regulating home for the aged facilities. Issuance of a regular license is recommended.

Visit Reason
The inspection was conducted as part of the original licensing study for Traditions of Saginaw - West to determine compliance with applicable licensing statutes and administrative rules for a home for the aged.

Findings
The facility was found to be in substantial compliance with licensing requirements after an on-site inspection identified some non-compliance items which were addressed through corrective action plans and documentation. A temporary license with a maximum capacity of 100 beds was recommended and issued.

Deficiencies (5)
Exhaust ventilation in some required rooms was connected to light switches and not functioning continuously.
Refrigerator and freezer thermometers indicated temperatures above required limits.
Food record maintenance was not properly kept by Chef Jason Brenay.
Building, equipment, and furniture were not kept clean and in good repair, specifically the game room ceiling was not fully repaired.
Uncertainty about fire safety certification approval for the facility sauna.
