On-site complaint investigation of complaint #115695 combined with an annual compliance inspection at an Assisted Living Home, conducted 24 March 2025.
Complaint Details
The following deficiencies were found during the on-site compliance inspection and investigation of complaint #115695 on March 24, 2025.
Findings
This inspection found ten deficiencies related to failure to maintain required resident emergency forms, inadequate opioid quality management and documentation, missing personnel records, incomplete tuberculosis screening documentation, missing medical service documentation on acceptance, improper acceptance of a non-ambulatory resident without required authorization, and deficiencies in medication administration and controlled substance management.
Deficiencies (10)
A.R.S. § 36-420.04.C — The facility failed to maintain a standardized emergency responder form for each resident including required patient information, posing a risk in emergencies.
R9-10-120 — The manager failed to administer a quality management program that included surveillance and monitoring of adherence to opioid policies and procedures.
R9-10-120 — The manager failed to ensure authorized individuals documented the resident's need for opioid medication and monitored the effect before and after administration.
R9-10-806 — The manager failed to have a personnel record for an employee, risking resident health and safety.
R9-10-807 — The manager failed to ensure a resident provided documentation of freedom from infectious tuberculosis as required.
R9-10-807 — The manager failed to ensure required signed and dated documentation was submitted before or at acceptance indicating medical service needs.
R9-10-815 — The manager failed to ensure the facility did not retain a non-ambulatory resident without a signed and dated determination from a medical practitioner authorizing continued residency.
R9-10-816 — The manager failed to ensure medication was administered in compliance with orders and documented in the resident's record.
R9-10-816 — The manager failed to implement policies and procedures for discarding medication, resulting in storage of medication for a resident no longer at the facility.
