The Department completed a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies related to investigations of alleged abuse and neglect.
Complaint Details
The complaint investigation involved an allegation that a named resident was yelled at by an identified staff member. The investigation found that the staff member raised their voice to the resident, was written up, and allowed to return to work. The facility failed to report the incident to the hotline initially. The complaint number is 149130.
Findings
The follow-up inspection on 05/20/2025 found no deficiencies and confirmed that the previously cited deficiencies under WAC 388-78A-2371 were corrected. The facility met all requirements for investigating and documenting alleged or suspected abuse incidents.
