Incident reporting: The Residence filed 8 incident reports greater than 24 hours after the occurrence of the incident during the period reviewed.

Emergency call response: Documentation of the ALR management testing the system to ensure functionality as required by the Residence policy was missing for the calendar years reviewed.

Quality assurance program: Two records were without documentation to confirm the initial assessment was conducted by a nurse, and three records were missing documentation to assess the ability of the Resident to manage medication, including the ability to take medication on an as-needed basis. One resident had a [redacted] missing in the LMA locked medication box missing the date [redacted] were opened.

Emergency drills: Documentation that all applicable staff from all shifts participated in elopement and evacuation drills was missing for the 2022 calendar year reviewed.
